Diponegoro National Hospital will offer a Booster Vaccination program (3^rd dose vaccine) using ASTRAZENECA and PFIZER vaccines which will be held at Diponegoro National Hospital Polyclinic from March 14 – March 18, 2022.

Requirements:

1. Adults 18 years and older
2. Specified for those receiving Sinovac, AstraZeneca or Pfizer vaccines on the 1^st and 2^nd vaccination
3. At least 3 months after completing your primary COVID-19 vaccination series
4. Bring your ID card, proof of the 2^nd vaccination and the vaccination registration card which is done through SSO or Victory
