Ketentuan Pemindahan Fasilitas Pelayanan Kesehatan (Fasyankes) Bagi Mahasiswa Angkatan 2020 Dan 2021 Universitas Diponegoro

Sebagai tindak lanjut pengumuman Rektor Universitas Diponegoro Nomor 2 / UN7.A1 / KM / I /2023, dengan ini kami sampaikan hal-hal terkait pemindahan Fasilitas Pelayanan Kesehatan (Fasyankes) ke Klinik Pratama Diponegoro I bagi mahasiswa angkatan 2020 dan 2021, Informasi selengkapnya bisa Download Disini

Tahapan dalam pemindahan Fasilitas Pelayanan Kesehatan yaitu :

1. Lakukan Pindah Fasyankes melalui JKN Mobile

  • Siapkan no BPJS, no HP dan email aktif (bagi yang belum punya akun)
  • Unduh Aplikasi JKN Mobile
  • Login dan Pilih Menu Perubahan Data Peserta
  • Klik Gambar panah pada fasilitas kesehatan tingkat 1, Klinik Pratama Diponegoro 1

2. Laporkan hasil perpindahan Fasyankes melalui SSO dengan cara :

  • Login SSO dengan akun mahasiswa masing-masing
  • Pilih Aplikasi FORM
  • Pilih Form Pindah Fasyankes

3. Upload surat keterangan melalui laman : https://linktr.ee/verifikasifaskes

Dalam hal mahasiswa mengalami permasalahan dalam pemindahan Fasyankes dapat menghubungi layanan Helpdesk di Hallo UNDIP
